Practice Closure for Staff Training
Just a reminder that the surgery will be closed for staff training from 13:00 – 18:30 on Thursday 12th March.
We will reopen as normal at 08:00 on Friday 13th March.
If you need urgent medical advice during this time, please contact NHS 111. GP cover has been arranged locally.

If your sickness absence is for 7 days of less, you do not need to provide a fit note to your employer so there is no need to complete this form. Your employer may ask you to complete a self-certification form instead.
If you have been off work sick for more than 7 days, please complete the sick note request form.
