Shared care planning for chronic diseases

We aim to involve our patients as much as possible with any decisions about their care. This involves providing information and guidance where appropriate, as well as treating patients individually and according to their own specific circumstances, priorities and goals.

To aid us with this we are introducing shared care planning for chronic diseases; we already offer choice and guidance for hospital referrals via choose and book, and we now have access to patient decision aids for certain medical conditions. These are online interactive tools to help a patient decide on what treatment best suits them for a condition where there may be more than one treatment available. Your doctor may signpost you to one of these patient decision aids but there is no barrier to anyone accessing them at any time.
